Annual Medical History Forms should be reviewed and updated as the portal indicates or when you know a significant change has occurred with your health. You will be able to gain access to this portal up to 48 hours after your initial request is approved.

Patient Portal Access (existing patients & new access only)
The first visit to our office is designed to get you better acquainted with all we have to offer as your dental provider. During this initial visit you will either meet with Dr. Leslie Ferrara or Dr. Maria Upshaw, as well as the rest of their devoted support team. Routinely, we reserve time to perform the necessary x-rays, which is often a full series of digital x-rays, along with oral images of the entire mouth, especially any suspicious areas. We offer a visual oral cancer screening to all patients, at no additional charge, although we urge each patient to take advantage of the latest technology for oral cancer detection, which is called VELScope VX. The VELScope VX screening cost is as low as $25, which is commonly not a covered benefit via dental insurance, unless you are over the age of 40.
Typically, we are able to present all treatment recommendations, including hygiene care, at this initial visit. The frequency in which you would return annually, to see your hygienist, is contingent upon your periodontal condition. The periodontal screening and its findings, will be fully explained during this visit. We encourage questions to further educate our patients and always do our best to deliver quality care and simplicity in such a complicated field.
Please take a moment, prior to your reservation, to tell us a bit about yourself and complete your health history. For this process, we ask that you click the link below and complete all four forms. This process streamlines and reduces your wait time. Please allow a minimum of 90 minutes for your initial visit with us.
So we may supply you with the most accurate information, we request that each patient supply their medical and dental insurance cards and any necessary updates, as soon as possible, to avoid an out of pocket expense. We strive to complete insurance verification, at no additional charge, 2-4 days prior to a patient visit. If benefits are unable to be verified prior to a patient arrival, anticipated costs would be out-of-pocket. To avoid this out-of-pocket expense, please supply all new or changed dental benefit information as soon it becomes available.

Please provide a few items in advance of your visit if you wish for our office to handle your insurance needs. You may email, text, or fax them to us - Please be sure to provide insurance information at least 2+ days prior to your visit to avoid an unnecessary out-of-pocket expense. New Patient Visits default to a one-time fee of $199 for those plans we are unable to verify or obtain benefits for. We will inform you prior to your visit if this becomes the case.

When using metered street parking, please visit http://us.parkmobile.com/ or download the ParkMobile app so you can pay right from your vehicle and replenish the meter right from the dental chair! (this is a FREE app and requires a Smartphone along with vehicle information)
Fifth Third offers valet for all of it's tenants and their guests. Valet is $3.50/hr and he accepts CASH only. The valet parking entrance is on Tampa Street, between Kennedy Blvd and Jackson Street.
You may also use any of the SevenOneSeven lots throughout downtown Tampa or the Fort Brooke Garage off Whiting Street.
